PRIVACY NOTICE FOR ISM PUBLIC NO CLAIM DISCOUNT (NCD) ENQUIRY
The personal data you have provided in the form (“Personal Data”)
will be processed for purposes of providing you the ISM Public NCD Enquiry services
and other related purposes (collectively, “Purpose”).
ISM also wishes to inform you that your Personal Data may be disclosed to third
parties, such as service providers for reasons relating to the Purpose.
You may request for access to or correction of your Personal Data or limit the processing
thereof at any time hereafter by submitting such request to ISM via email to
pdpo@ism.net.my. Any inquiries or complaints with respect to your Personal
Data should also be channeled to ISM in this manner.
Please note that it will be necessary for ISM to process your Personal Data for
the Purpose, without which ISM will not be able to provide you and you will not
be able to use the ISM Public NCD Enquiry services.

Data peribadi yang anda berikan di dalam borang ("Data Peribadi")
akan diproses untuk tujuan-tujuan memberikan anda perkhidmatan Pertanyaan NCD Am
ISM dan tujuan-tujuan lain yang berkaitan (secara kolektif, "Tujuan").
ISM juga ingin memaklumkan kepada anda bahawa Data Peribadi anda mungkin didedahkan
kepada pihak ketiga, seperti penbekal perkhidmatan atas sebab-sebab yang berkaitan
dengan Tujuan.
Anda boleh meminta akses kepada atau pembetulan terhadap Data Peribadi anda atau
mengehadkan pemprosesan tersebut pada bila-bila masa selepas ini dengan mengemukakan
permintaan tersebut kepada ISM melalui e-mel pada pdpo@ism.net.my.
Sebarang pertanyaan atau aduan berkenaan Data Peribadi anda juga harus disalurkan
kepada ISM melalui cara ini.
Sila ambil perhatian bahawa ia adalah perlu untuk ISM memproses Data Peribadi anda
bagi Tujuan, tanpanya ISM tidak akan dapat memberikan anda dan anda tidak akan dapat
menggunakan perkhidmatan Pertanyaan NCD Am ISM.
